This site is a confidential and secure, internet-based service that allows eligible members of The State of Tennessee Health Plans to obtain access to and organize their own health information. The website provides you with useful information about opportunities for improving the quality of your medical care that you can share with your doctor. The website is offered in connection with your health benefit plan through ActiveHealth Management and there is no separate charge to you for this service.

What Type of Information is Contained in this Website?

The website contains information about your health that your health plan collects and maintains in connection with providing your health benefits. This information primarily consists of claims for reimbursement submitted by your health care providers. You can also add information to the website about your own health conditions and medical treatment. We encourage you to add your own information as this makes this website a more valuable tool for you.
This website does not contain all of the information about your health. Among other things, it does not include information relating to sensitive medical conditions such as HIV/AIDS, treatment for sexually transmitted diseases, certain mental health care, drug or alcohol abuse, or family planning. It is important that you and your health care providers understand this limitation of this website and not use the website as the sole source of information about your health conditions or medical treatment.

How May The Health Information Contained Within this Website Be Used by ActiveHealth?

The website serves as both a repository of your health information as well as a tool to inform you and your physicians about opportunities to improve your health. As new data is received, it is scanned in order to identify potential ways in which your health care may be improved. In addition, the claims data present in this website, as well as any information you provide on your own, may be utilized by ActiveHealth in connection with any other ActiveHealth programs or services that may be provided to you through your health plan. Such programs or services may include disease management services and/or health advocacy services.

What Type of Clinical Messages are Issued Through this Website?

This website uses a proprietary software system, CareEngine®, to identify ways in which your health care might be improved. The CareEngine matches information in this website about your health and treatment against a set of generally accepted clinical ""best practices"" derived from the medical literature. For example, the CareEngine might identify that you have recently filled prescriptions for two drugs that should not be taken at the same time.

The clinical messages, called Health Action Alerts, issued by the CareEngine are for informational purposes only. These messages do not constitute, and are not intended as, a substitute for medical advice. The messages may not be appropriate for your particular medical condition because the messages are based exclusively on information in this website, which may be incomplete. Your doctor and other health care providers are solely responsible for your medical treatment.

The Health Action Alert messages are delivered to you in order to provide you with information you can discuss with your doctor. We encourage you to speak with your doctor about the Health Action Alerts, so you and your doctor can determine whether any change in your medical care is warranted. You should never rely on these messages as the sole basis for changing your medical treatment. As always, if you think you have a medical emergency, you should immediately call your doctor or 911.

How does ActiveHealth use and disclose the information in this Website?

The information in this website is considered ""protected health information"" and its use and disclosure is restricted under the Health Insurance Portability and Accountability Act (""HIPAA""). As a result, ActiveHealth may use and disclose personal information in the course of running our health business or for purposes of payment or treatment including, but not limited to: conducting operational activities such as quality assessment and improvement; licensing; accreditation by independent organizations; performance measurement and outcomes assessment; health services research; disease management, case management and care coordination; data and information systems management; customer service; to facilitate the sale, transfer, merger or consolidation of all or a part of ActiveHealth Management or Aetna with another entity, to facilitate payment for your covered services; to conduct utilization and medical necessity reviews; responding to complaints, appeals and requests for external review; and disclosing information to doctors, dentists, pharmacies, hospitals and other health care providers who take care of you. We may disclose personal information to other covered entities or business associates of those entities for treatment, payment and certain health care operations purposes.

Active Health will not disclose Protected Health Information unless you consent to the disclosure or it is otherwise authorized by HIPAAand the Business Associate Agreement between the State and ActiveHealth. If you have provided us with authorization to disclose your Protected Health Information, you may revoke it at any time if we have not already acted on your authorization.

In all situations other than those described above, ActiveHealth will ask for your written authorization before using or disclosing personal information about you. If you have given us an authorization, you may revoke it at any time, if we have not already acted on it.

How is the Security of this Website Maintained?

ActiveHealth employs administrative, physical and technical safeguards to maintain the security of this website. We believe these safeguards are consistent with applicable law and are generally effective in preventing unauthorized access to your information.

You also play an important role in ensuring the security of this website. As part of this website registration process, you choose a password for accessing the website. You should not share your password with anyone. You should understand that if another person has your password they will be able to obtain access to this website as if they were you.

May I Change or Delete Information in this Website?

You may change or delete any information in this website that you have provided.

You may discontinue the use of this website at any time by calling member services toll-free number contained within the website. If you have authorized health care providers to access your website, this access will be automatically terminated once we receive your notice of discontinuance.

If you discontinue use of this website, ActiveHealth will save all of the information in the website for a period required by applicable law. You may request a copy of this information during the period when it is retained by ActiveHealth.

Which Other Legal Rules Apply to this Website?

The obligations of ActiveHealth to provide a website are found in Section A.14 of the Primary Population Health contract and Section A.11 of the Weight Management contract. Your use of this website operates as a waiver of all claims against ActiveHealth for consequential, special, and indirect damages to the extent authorized by Tennessee law.
